First and Last Name (capitalized)

Anniversary Date
Membership Level (displays all they are members of - if multiple)
Last Visit
Top 5 Frequent Items Purchased in Past 90 Days
Last 3 General Notes Stored on Profile
Only fields with data are displayed.
